This event happened in a South African aquarium where they were keeping a pregnant Ragga Tooth Shark. The diver was trying to sedate the shark so that they could do a proper examination on it, turns out that the shark didn't appreciate their work. The shark turned around and tore this guy's arm up pretty terrible. The diver was able to make a full recovery and holds no ill will towards the animal, but he is going to make sure that this aquarium starts stocking shark-suits to avoid this in the future.
