He is about to learn why dentists are paid that much. If his teeth are that bad that they need to be pulled then his hygiene isn’t all that important to him. Using a pair of dirty pliers to yank the tooth out is probably not that worst thing he has down to his mouth.

The face of instant regret. A trip to the dentist might cost more but at least they will give you a subscription to some awesome painkillers after, Advil isn't going to help him a ton after this.
