Distressing footage captures the last moments of a skydiver who, in April 1988, discovered mid-air that he had jumped from a plane without his parachute.
Veteran skydiver Ivan Lester McGuire, filming a skydiving lesson, moved ahead of the student and instructor to document the experience on camera.
It was only after Ivan took the leap that he realized he had forgotten his parachute on the plane.
Subsequently, he recorded his tragic descent from 10,500 feet in the air, leading to a fatal outcome.
The last words captured on the recording from Ivan were: "Oh my god, No!"
Nancy Fayard, the spouse of Paul Fayard, the owner of the parachute center, was quoted in the news during that period.
She told AP: "No one was aware that he got on the plane without a parachute. Of course, no one knew or they would have stopped him."
Officials thought the skydiver may have been fatigued or distracted because the incident was unexpected given that he had completed over 800 successful jumps.
"We haven’t gotten all the facts yet. But there is a regulation, No. 105, that states that the pilot must check - well, no one may jump unless the pilot checks the parachutes,” Walter Rigsbee, an FAA inspector, continued.
Even after the tragedy, pilot Mark Luman was questioned by officials, who promptly excluded suicide from their inquiry.
In the end, it was determined that Ivan had simply forgotten his parachute, and his death was an accident.

He passed away at 35 years old. According to AP, he intended to pursue a profession as a skydiving photographer.
The body of the skydiver was discovered 1.5–2 kilometers from the airfield where the aircraft took off.
In 1999, Joan Murray, a fellow skydiver, came dangerously close to experiencing the same terrible outcome when her parachute failed.
Joan pulled on the parachute toggle in a panic and it opened gradually.
Nevertheless, because Joan had been spinning, it got knotted quite fast.
Despite having a 50% chance of surviving the event, Joan, who was 47 at the time, fell on a mound of red ants.
Joan's life was saved by a pile of red ants, despite the fact that most people wouldn't volunteer to sit on one.
When Joan was attacked by the ants after she fell, the sting of their venom caused her body to experience an adrenaline rush, which is what kept her heart pumping.
She was later saved and taken to a nearby hospital, where it was discovered that she had multiple fractured teeth and damaged bones.
Even though Joan suffered severe injuries in the end, it was still miraculous that she survived to tell the story.
